Successful intracardiac electrical conversion of atrial flutter in patients with complex congenital heart disease.

چکیده
Sixteen patients presenting on 21 occasions with atrial flutter in association with complex congenital heart disease were treated by intracardiac stimulation techniques combined with activation mapping. Nineteen episodes of atrial flutter were successfully converted to sinus rhythm. In the remaining two episodes atrial fibrillation was induced with spontaneous conversion to sinus rhythm within 12 hours in one episode and immediate DC cardioversion to sinus rhythm in the other. Intracardiac stimulation techniques were highly successful in this group and allowed reliable conversion to sinus rhythm without general anaesthesia and high energy cardioversion. In patients with atrial flutter associated with congenital heart disease intracardiac stimulation techniques should be tried first.

Right Atrial Stimulation
Pervenous electrical stimulation of the right atrium was successful in converting 15 episodes of supraventricular tachyarrhythmia in 9 of 15 patients. This included 13 conversions of atrial flutter, one each of atrial tachycardia and junctional tachycardia. The procedure was performed at the bedside, without complication', using intracardiac electrography to direct catheter position. Indirect e...

Introduction: Most current evidence suggests the risk of left atrial and or auricular thrombi(LA &LAA) in patients with atrial flutter rhythm is similar to patients with atrial fibrillation, but there is still uncertainty in the risk level and the patient's need to receive anticoagulant therapy. Different studies report various frequencies for atrial thrombi and left atrial spontaneous echo-con...
متن کاملFontan conversion with arrhythmia surgery.
OBJECTIVE Hemodynamic abnormalities and refractory atrial arrhythmias in patients late after the Fontan operation result in significant morbidity and mortality. We reviewed our experience with Fontan conversion and concomitant arrhythmia surgery. METHODS Between January 1996 and February 2004, 16 patients underwent Fontan conversion and arrhythmia surgery. Mean age at the initial Fontan opera...
متن کاملImportance of atrial flutter isthmus in postoperative intra-atrial reentrant tachycardia.
BACKGROUND In survivors of congenital heart surgery, intra-atrial reentrant tachycardia (IART) often develops. Previous reports have emphasized the atriotomy scar as the central barrier around which a reentrant circuit may rotate but have not systematically evaluated the atrial flutter isthmus in such patients. We sought to determine the role of the atrial flutter isthmus in supporting IART in ...
متن کاملA Case Report of Neonatal Atrial Flutter with Poor Feeding and Tachypnea
Background: Arrhythmias in neonates are not common and usually affect newborns with a normal heart or those that suffer from structural heart disease. A malignant type of arrhythmias is supraventricular tachycardia. Meanwhile, one uncommon type of supraventricular arrhythmias is atrial flutter (AFL), which is founded upon reentry mechanisms in the atrium. The AFL may result in heart failure or ...
